Through strategic and compassionate assistance, we aim to ensure that individuals can access the necessary resources to navigate their treatment effectively, fostering a pathway for a successful return to the workforce and restoring financial stability for themselves and their families post-treatment.
Qualifications to apply:

Residents of Albany, Columbia, Greene, Fulton, Montgomery, Rensselaer, Saratoga, Schenectady, Schoharie, Warren, or Washington counties

Be a person having a confirmed blood cancer diagnosis from an accredited medical center referred to us by that institution's social worker or member of its medical staff

Signed by the patient's treatment center’s Clinical Licensed Advocate

Signed and properly attested by the Patient/Guardian
-
Be at or below an annual gross income level of 500% of Federal Poverty Guidelines

If your circumstances meet our qualifications, please ask your treatment provider to apply on your behalf. Participating treatment centers are NYOH, AMC, and Dana Farber. If you are from another treatment center, please have your social worker contact us for the application.
